What is the Next Step? Once there is reason to believe that you or someone that you love has a life-limiting illness, it is time to make a Hospice referral. The easiest way to do this is to call the Hospice admissions department at 800-717-3811. The admissions coordinator will then telephone your physician to verify that the patient is appropriate for Hospice care. Once the doctor verifies that the patient's physical condition warrants hospice care, the admissions coordinator will set up an appointment with a nurse and/or social worker to do an admission visit. During the admission visit hospice services are described, questions are answered, and consent forms are signed. If the patient elects hospice care, a physical assessment will also be completed. Once a patient signs up for Hospice, there is no lengthy waiting period, the benefits begin immediately. Physical, emotional, spiritual, and financial relief are only a phone call away.
